Installment Agreement Request
Form 9465 is a request to pay a federal tax debt in monthly installments. You use it when you can't pay everything you owe by the due date.
An approved agreement does not stop the clock. The instructions say interest and any applicable penalties continue to accrue until the balance is paid in full. What the agreement gives you is time and a clear schedule.
Many people can apply online instead. The IRS online payment agreement is available for balances of $50,000 or less and carries a lower fee, so check that option first.

Your address and whether it is new.

Used for closed businesses.

The total owed, any payment made with the request and the remaining balance.

The amount you offer. If it is less than line 10 and you can't increase it, the instructions say to attach Form 433-F.

A day from the 1st to the 28th of each month.

Routing number, account number and the low-income reimbursement checkbox.

Requires Form 2159.

Required after a default within 12 months for balances over $25,000 up to $50,000 when the proposed payment is too low.

Balances of $50,000 or less can be arranged at IRS.gov/OPA with a lower fee.

Lines 5 through 7 total what you owe. Line 8 is any payment you send with the request, and line 9 is the remaining balance.

The instructions describe agreement types: guaranteed for $10,000 or less within 3 years, streamlined for $25,000 or less, or $25,001 to $50,000 with direct debit or payroll deduction, paid within 72 months or by the collection statute expiration date, whichever is less.

Choose the 1st through the 28th.

A nine-digit routing number and an account number of up to 17 characters. Both spouses sign on a joint return. You can fill the form free on this page.

Direct debit approval requires your signature, and your spouse's on a joint return.

You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Attach Form 9465 to your return if you file with it. Otherwise mail it alone to the IRS address for your state, using the table in the instructions. Taxpayers outside the 50 states use a separate Austin, Texas address.
File Form 9465 by itself at the address in the table. The IRS usually responds within 30 days, or longer for returns filed after March 31.
The instructions list fees by method. Direct debit and online requests cost less than other methods, and a reduced fee exists for low-income taxpayers. The current amounts are in the instructions.
A Notice of Federal Tax Lien generally isn't filed for guaranteed or streamlined agreements, but it may be in certain situations.
Missed payments can end the agreement and lead to levies. Appeals may be available through the Collection Appeals Program.
Requesting a payment too small to pay the balance in time, leaving off bank details, forgetting that interest keeps running and not sending the form to the right address are the common errors.
