Supporting Documents To Prove Head of Household Filing Status (Spanish Version)
Form 886-H-HOH (SP) is the Spanish version of Form 886-H-HOH, Supporting Documents To Prove Head of Household Filing Status. The PDF is printed in Spanish under the title Documentos de Apoyo Para Demostrar el Estado Civil de Cabeza de Familia, for taxpayers who read Spanish. The English edition is Form 886-H-HOH.
It is a one page IRS table of photocopies. The form says Head of Household status may apply when three requirements are met: the marriage requirement, the qualifying person requirement and the cost of keeping up a home requirement. For each one it lists the documents to send for tax year 2019, such as a divorce decree, a lease agreement, school or medical records, and rent receipts or utility bills.
The top of the form has spaces for the taxpayer's name, taxpayer identification number and tax period ending. You can fill them out free in your browser on this page and download the result without an account.

The marriage requirement, in three rows: single, divorced or legally separated, and married with the spouse not living in the home during the last 6 months of tax year 2019.
A child (including an adopted child or one whose adoption is pending), a brother or sister, stepbrother or stepsister or any of their descendants, or a qualifying foster child, for whom a dependency exemption can be claimed and who lived in the home more than half of 2019. The form says temporary absences, such as time at school, count as time lived in the home, and that a married child has to be a dependent.
Birth certificates or other official birth documents, marriage certificates, letters from an authorized adoption or placement agency, or a court document. The form says to send these only for a qualifying child who is not a natural or adopted child.
School, medical, daycare or social service records, or a letter on official letterhead showing names, addresses and dates. A letter from a relative who provides daycare needs at least one additional letter.
For paying more than half the cost of keeping up the home for 2019: rent receipts, utility bills, grocery bills, real estate tax bills, mortgage interest statement, upkeep and repair bills, property insurance and other household expenses.
Enter the taxpayer's name, taxpayer identification number and the tax period ending in the three spaces at the top.
Pick the row that matches: soltero, divorciado o legalmente separado, or casado with the spouse living elsewhere for the last 6 months of 2019. Single filers move on to the next two requirements.
Photocopy the complete decree or agreement, or the lease agreement, utility bills, or a letter from the clergy or social services.
Add relationship papers only if the child is not a natural or adopted child, plus records showing the child lived in the home more than half the year. The form says to send as many documents as necessary.
If a letter comes from a relative who provides daycare, the form asks for at least one additional letter.
