8809-EX
Request for Extension of Time To File an ExSTARS Information Return
Form 8809-EX is the request for a 30-day extension of time to file an Excise Summary Terminal Activity Reporting System (ExSTARS) information report. The two reports it covers are Form 720-TO, Terminal Operator Report, and Form 720-CS, Carrier Summary Report. One Form 8809-EX can cover both reports as a combined filing.
The instructions say the automatic extension is 30 days from the original due date, and that all initial 30-day requests are honored except those without a reason. One additional extension of not more than 30 days can be requested with a second Form 8809-EX before the first extension period ends. The instructions say additional requests are not automatically granted.
This edition is dated January 2014 and prints its instructions on page 2. You can fill out Form 8809-EX free in your browser on this page and download the result without an account.

The automatic extension is 30 days from the original due date. A second Form 8809-EX filed before the first extension ends can ask for one more extension of not more than 30 days.
If the IRS denies a request, it sends a letter to the address on record. The instructions say that without a denial letter the request may be considered approved. After a denial, the return is due by the required filing date or within 1 business day from the official denial date, whichever is later.
The instructions say filing required information returns late without an approved extension may bring a late filing penalty based on when the correct return is filed. Failure to update a change of address may also result in a late filing penalty.
