Form 4547 (sp) is the IRS Spanish version of Form 4547, Trump Account Election(s). Its Spanish title is "Elección(es) de Cuenta Trump," and the parts, checkboxes and consent are written in Spanish. The English edition is Form 4547. The IRS About page says taxpayers use this form for elections to open an initial Trump account and to elect to request a one-time $1,000 pilot program contribution.
The form says that for each eligible child for whom an account is opened, Parts I, II and IV are completed, and that the box on line 7 of Part III is checked for each child eligible for the $1,000 Pilot Program Contribution (Aportación del programa piloto). The parent, guardian or other authorized individual listed in Part I becomes the person responsible for the Trump account. The form points to www.irs.gov/Form4547SP for instructions and the latest information.
