Form 433-B: Collection Information Statement for Businesses

Form 433-B, Collection Information Statement for Businesses, is an IRS form on which a business reports its financial picture: business details, the people who own and run it, other financial information, assets and liabilities in the United States and abroad, and a monthly income and expense statement.

The form says to complete all entry spaces with the current data available or "N/A" (not applicable), and that failure to complete all entry spaces may result in rejection of the request or significant delay in account resolution. It is signed under penalties of perjury by an officer, partner or LLC member. After the IRS reviews it, the form says the business may be asked to verify the assets, encumbrances, income and expenses it reported.

This is the June 2026 revision (Rev. 6-2026). The PDF can be filled out free in the browser on this page and downloaded without an account.
