Status of the Taxpayer Levy (Spanish Version)
Form 15104 (SP), Estado del Embargo del Contribuyente, is the Spanish version of Form 15104, Status of the Taxpayer Levy. It is a one-page form printed in Spanish and dated Junio de 2017. It is completed by an employer, not by the taxpayer. The top asks for the employer's name and employer identification number (EIN), then the taxpayer's name and Social Security number (SSN).
The form says to complete it if the required amount was already sent or no money is owed to the taxpayer, and to return it in the enclosed envelope, or to give the information by phone at the number on the attached letter. It then lists three statements written as the employer: the amount was sent, no money is owed to this taxpayer, or the taxpayer is no longer employed.
The English edition is Form 15104. You can fill out Form 15104 (SP) free in your browser on this page and download it without an account.
