Request for Tax Deferment (Spanish Version)
Form 15109 (sp), Solicitud de aplazamiento de impuestos, is the Spanish version of Form 15109, Request for Tax Deferment. Its two sections and its Privacy Act notice are printed in Spanish, and the header shows the date enero de 2023. The Privacy Act notice says the main use of the information is to determine eligibility for tax deferment based on service in a combat zone or contingency operation, under section 7508 of the Internal Revenue Code.
The form asks for the person's name, Social Security number and the tax form and year, the organization, employer or military department, and the dates of entry and exit for each combat zone or other area. Section 2 lists the supporting documents to include. The form says the Document Upload Tool it names is available in English, and that notice CP04 explains the tool.
The English edition is Form 15109. You can fill out Form 15109 (sp) free in your browser on this page and download it without an account.
