Provide a Social Security Number (SSN) for Adoptive Child (Spanish Version)
Form 15101 (sp) is the Spanish version of Form 15101, Provide a Social Security Number (SSN) for Adoptive Child. The PDF is in Spanish, under the title "Proporcione un Número de Seguro Social (SSN) para el hijo adoptado", and is dated October 2017 (Rev. 10-2017). Page 1 is the form and page 2 holds its instructions, also in Spanish.
The instructions say to use it only to tell the IRS the SSN of your adopted child when the IRS had given that child an adoption taxpayer identification number (ATIN). Once the adoption is final, they say not to keep using the ATIN and to report the child's SSN on this form instead. The form does not get the child an SSN: the instructions say only the Social Security Administration (SSA) assigns one, through Form SS-5 (sp).
The adoptive parents fill it out, sign it and send it to the IRS in Austin, Texas, by mail or fax. The English edition, Form 15101, has its own page. You can fill out this Spanish PDF free in your browser on this page and download it without an account.
