Adoption Taxpayer Identification Number (ATIN) Extension Request (Spanish Version)
Form 15100 (sp) is the Spanish version of Form 15100, Adoption Taxpayer Identification Number (ATIN) Extension Request. Its Spanish title is "Solicitud de prórroga del Número de identificación del contribuyente en proceso de adopción (ATIN)". The fields and directions are in Spanish. The header "Department of the Treasury - Internal Revenue Service" and the footer are in English.
It is a one-page form for adoptive parents ("padres adoptivos") who have an ATIN for an adopted child. They check one of three circumstances: they received a Social Security number for the child, they need a one-year extension of the ATIN, or they could not get a Social Security number for the child. The form also asks for contact details, whether the adoption is final or pending, and the parents' original signatures.
The top of the form says to complete it and send it to the IRS by mail or fax, with addresses in Austin, Texas. No separate instructions are printed or linked. You can fill out the PDF free in your browser on this page and download it without an account.

"Indique cuál de las siguientes circunstancias le corresponde": three boxes, each with the adopted child's name and the SSN or ATIN assigned.
A "Razón" space under the extension box and under the box for not getting an SSN.
"Estado de la adopción": boxes for final ("La adopción es definitiva") or pending ("La adopción está pendiente"), and a line stating that documents signed by the agent or child placement agency are attached.
Name typed or printed, signature and date for each adoptive parent.
The labels are in Spanish. The items below give their English meaning.
Enter each name with its taxpayer identification number. The form says to write a new address only if the address changed, then add phone numbers and the best time to call.
Check the box that applies and enter the adopted child's name.
For an SSN received, write the Social Security number assigned. For an extension, or when no SSN was obtained, enter the ATIN assigned.
Write the reason in the "Razón" space under the box that was checked, when that box has one.
Check final or pending. The next line says documents signed by the agent or child placement agency are attached.
Each adoptive parent types or prints a name, signs with an original signature and adds the date.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
