Form 15080 (ja) in Japanese: Consent to Disclose Tax Return Information to VITA/TCE Sites

Form 15080 (ja) is the Japanese version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS PDF is written in Japanese for taxpayers who read Japanese and are offered this consent at a VITA or TCE site. The English edition is Form 15080. This page describes the Japanese edition in English.

The consent is for Global Carry Forward, a term the Japanese text keeps in English with a Japanese gloss. It says Global Carry Forward lets TaxSlayer LLC, the provider of the VITA/TCE tax software, make the taxpayer's return information available to any volunteer site in the IRS VITA/TCE program that the taxpayer picks for next filing season's return. This October 2025 edition says the consent is valid until November 30, 2027.

The form says completing it is not required to use the site's tax return preparation services. The primary taxpayer and any secondary taxpayer print their names, sign and date. You can fill it out free in your browser on this page and download the result without an account.

What's on Form 15080 (ja)

Federal Disclosure

Except as the law allows, the site does not disclose return information to third parties for purposes other than preparing and filing the return without consent. Once disclosed, federal law may not protect it from further use or distribution.

Terms: Global Carry Forward

How TaxSlayer LLC makes the return information available to a participating volunteer site, so this year's data appears on next year's return wherever this year's was filed, and the November 30, 2027 end date.

Information disclosed

Name, address, date of birth, phone number, SSN, filing status, occupation, employer's name and address, income, deductions and refund amounts, and the names, SSNs, dates of birth and relationship of dependents on the return.

Limits on duration and scope

Two statements that the taxpayer does not wish to limit the consent to a date before November 30, 2027 or to a narrower scope. Each says a taxpayer who does will refuse consent.

Consent and signature lines

The statement that the taxpayer has read the information above and consents, then lines for the primary and secondary taxpayer's printed name and signature, each with a date.

TIGTA contact

A closing note on reporting improper disclosure or misuse to the Treasury Inspector General for Tax Administration (TIGTA) at 1-800-366-4484 or through the TIGTA web address the form prints.

How to fill out Form 15080 (ja)

  1. 1

    Read the Federal Disclosure

It explains what the site may do with return information without consent, and that a consent with no period set is valid for one year from the date of signing.

  2. 2

    Read the terms

Review what Global Carry Forward does, which information it covers and the November 30, 2027 end date.

  3. 3

    Check the limitations

The form says a taxpayer who wants an earlier end date, or a narrower scope, will refuse consent rather than sign.

  5. 5

    Secondary taxpayer

A secondary taxpayer prints the name, signs and dates the second line.

Consent terms, timing and contacts

Optional

The form says it is not required to use the site's tax return preparation services, and a consent obtained by making those services depend on it is not valid.

How long it lasts

The terms say the consent is valid until November 30, 2027. A consent with no period set is valid for one year from the date it is signed.

PIN entry

By consenting, the taxpayer authorizes the preparer to enter a PIN in the tax preparation software on the taxpayer's behalf to confirm consent.

Stays with the site

The form prints no IRS mailing address. It is written in the voice of the preparation site, and the taxpayer has the right to one signed copy.

Improper disclosure

If return information was disclosed or misused without permission or in a way the law does not allow, the form says TIGTA can be contacted at 1-800-366-4484.

Who uses Form 15080 (ja)?

  • Taxpayers who read Japanese

The form says federal law requires that the taxpayer be given this consent form, and that filling it out is not required to use the site's tax return preparation services.

  • Secondary taxpayers

A second line holds the secondary taxpayer's printed name, signature and date. The consent statements are written for "I/we, the taxpayer."

  • VITA/TCE sites and preparers

The site speaks as "we" throughout. By consenting, the taxpayer authorizes the return preparer to enter a PIN in the tax preparation software on the taxpayer's behalf.

  • TaxSlayer LLC

Named in Latin letters as the provider of the VITA/TCE tax software that makes the information available to the volunteer site the taxpayer selects.

  • Not needed for this year's preparer

The form says no consent is needed for the VITA/TCE partner preparing this year's return. Global Carry Forward helps only at a different VITA or TCE partner using TaxSlayer next year.

A VITA or TCE site hands the form to the taxpayer, and the taxpayer decides whether to sign it. The form prints no IRS address.
