Intake/Interview and Quality Review Sheet (Tagalog Version)
Form 13614-C (tl) is the Tagalog version of Form 13614-C, the Intake/Interview and Quality Review Sheet for the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program. Its questions are written in Tagalog under the title "Papel ng Pagtanggap/Panayam at Pagsusuri ng Kalidad", so a Tagalog-speaking taxpayer can give the intake answers before an IRS-certified volunteer prepares the return. Some terms, such as Social Security card and identity protection PIN (IPPIN), stay in English.
This edition is dated October 2025 (Oktubre 2025) and asks about 2025. It says to complete pages 1 to 5 and lists what to bring: tax information such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN letters for everyone on the return, and picture ID. Columns labeled for the certified volunteer are filled in by the volunteer.
Form 15080 (tl), a Tagalog consent for Global Carry Forward, is the last page. The English edition is Form 13614-C. You can fill out this PDF free in your browser on this page and download it without an account.

Answer on the left side of pages 2 and 3 and check only the boxes that apply to you or your spouse. The volunteer completes the right side.
Page 4 is optional. Form 15080 (tl) says you are not required to complete it to receive the return preparation services.
The form says you are responsible for the information on your return, and to ask the IRS-certified volunteer preparer if you have questions.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The sheet is used with the IRS-certified volunteer at the VITA or TCE site. The only IRS address it prints is for comments on the time estimates.
The form prints no deadline. Its questions refer to 2025.
The privacy notice says giving the information is voluntary, but without it the IRS volunteers may not be able to help prepare and file the return.
The form says to report unethical behavior to the IRS by email at [email protected].
If signed, Form 15080 (tl) lets TaxSlayer LLC make your return data available to any VITA/TCE site using TaxSlayer next filing season. It says the consent is valid until November 30, 2027.
