Form 13614-C (vie): Intake/Interview and Quality Review Sheet, Vietnamese Version

Form 13614-C (vie) is the Vietnamese version of Form 13614-C, the Intake/Interview and Quality Review Sheet used in the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program. Every question in the PDF is in Vietnamese, under the title "Tờ Tiếp Nhận/Phỏng Vấn và Duyệt Xét Phẩm Chất", so a taxpayer who reads Vietnamese can answer the intake questions before an IRS-certified volunteer prepares the return.

This is the October 2026 edition (Tháng 10 năm 2026), and its questions ask about 2026. The opening checklist says to complete pages 1 to 4 and to bring tax information such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN letters for everyone on the return, and a picture ID such as a valid driver's license. Columns marked for the certified volunteer are filled in by the volunteer.

The last page is Form 15080 (vie), a Vietnamese consent for Global Carry Forward of return data. The English edition is Form 13614-C. You can fill out this PDF free in your browser on this page and download it without an account.

Columns headed "Phải do tình nguyện viên được chứng nhận hoàn tất" (to be completed by the certified volunteer) cover the household table, the income to include and the expenses and events to report.

  • Who else may see it

The privacy notice says the information may go to others who coordinate VITA/TCE staffing, outreach and related activities, and that the IRS discloses return information only as 26 U.S.C. section 6103 provides.
