Request for Batteries and Accessories
VA Form VA2346a is the U.S. Department of Veterans Affairs Request for Batteries and Accessories. VA's form page says to use it to order hearing aid batteries and accessories, and lists it under health care.
The form is one page. It asks for your name, address, email, whether the address is new permanent or new temporary, the last four digits of your Social Security number and the date. A table then asks for a six month supply of batteries for each device by brand name, model, serial numbers and battery type or size, with a separate space for accessories, which the form says are shipped separately from batteries.
The form is mailed to the Denver Acquisition & Logistics Center, and VA's page says the request can also be submitted online. You can fill out the PDF free in your browser on this page and download the result without an account.
