VA Fiduciary's Account (Fillable)
VA Form 21P-4706b is the U.S. Department of Veterans Affairs VA Fiduciary's Account. It is not filed by the beneficiary: the instructions say Items 1 through 8 are completed by the fiduciary and returned to the VA Fiduciary Hub. The form says the fiduciary accounts for all VA funds received on behalf of the beneficiary and keeps receipts and other documentation of expenses, because VA may need to examine them during the audit of the accounting.
The form is two pages. Page 1 is the statement of account: money received, money spent, funds under management and assets at the end of the period. Page 2 holds background questions for individual fiduciaries and a certification of U.S. savings bonds.
