Veteran Patient Statement
VA Form 10-0246 is the U.S. Department of Veterans Affairs Veteran Patient Statement. It is a one page account statement for a patient. It shows the patient name, account number and statement date, an account summary of the previous balance, payments received, new charges and balance, and a table of charges with a description, amount and billing reference for each.
The bottom of the page is a payment coupon. The form says to detach the coupon when paying by mail or in person, and not to include any correspondence with the payment. It lists payment online at www.pay.gov, by mail, in person at any VA Medical Center and by phone at 1-888-827-4817.
The statement fields, such as the account summary and the local VA's message, describe the account. The coupon fields marked with an asterisk are the ones the form asks a credit card payer to complete. VA's page lists the form under VHA. You can fill out the PDF free in your browser on this page and download the result without an account.
