VA Police Officer Pre-Employment Screening Checklist
VA Form VA0120 is the U.S. Department of Veterans Affairs VA Police Officer Pre-Employment Screening Checklist. The form says the information is necessary to accomplish the pre-employment screening to determine the qualification and suitability of the applicant to be hired as a VA police officer.
It is a VA staff form, not an application. Part I identifies the applicant, and the other parts are labeled for the facility's Chief of Police and for Human Resources Management to complete: arrest record checks, telephone checks with past employers, the basis for qualification and a clarifying statement.
The form is two pages and prints OMB Approved No. 2900-0524. You can fill out the PDF free in your browser on this page and download the result without an account.

A check of the criminal record repository for the applicant's home of record for the past five years, chosen from drop down boxes, and a check of the F.B.I. National Crime Information Center. Each is marked No Record or Record, with charges and disposition explained in Part V.

Ten questions for up to three employers in columns A, B and C. The instructions say to determine the quality of work history for all employment within the past 5 years and to verify all employment used for qualification regardless of age.

Acceptable job experience meeting GS-083 minimum qualification requirements, acceptable substitute education shown by a transcript copy, or years of active military service in a qualifying specialty, with codes listed for the Army, Marine Corps, Air Force, Navy and Coast Guard.
Space to explain entries in Parts II, III and IV, followed by the Human Resources Management Service certification, signature and title of the verifier, and date.

The Chief of Police checks the criminal record repository and the National Crime Information Center, marks the results and signs and dates the Part II certification.
Human Resources Management completes the Part III questions for each employer. The form says all indicative negative responses are documented in Part V. The inquirer signs with their title and the date.
Mark experience, substitute education or military service in Part IV. When military service is used, the form says the specialty is verified by review of DD Form 214 or another official document.
Add any Part V statements and the verifier's signature. The PDF has buttons to submit by email to local HR, to print the form for shipment and to submit via email to LETC.

The PDF footer reads VA FORM APR 2014 0120. It carries OMB Approved No. 2900-0524 and a respondent burden of 10 minutes. VA's form page lists a form revision date of October 2013.
Part II, Arrest Record Checks and Certification, is marked "By Chief of Police" with the purpose code Criminal Justice Employment. The Chief of Police signs a certification about the candidate's experience.
Parts III and IV each say Human Resources Management must complete the section: the telephone checks with past employers and the basis for qualification.
At the end of page 2, Human Resources Management Service certifies that the applicant is suitable and meets current qualification standards for the GS-083 Police Officer series. The verifier signs with their title and the date.
Part III records answers from a named contact at each employer, such as the applicant's position, dates of employment, quality of work, honesty and character, and whether the employer would rehire the applicant.
Part I lists the police officer applicant's name, Social Security number, date of birth and sex, on page 1 and again on page 2. The checklist is about the applicant rather than filed by the applicant.
