Disposition of Body
Standard Form 523-A (SF 523-A), Disposition of Body, is a one-page medical record printed as STANDARD FORM 523-A (REV. 12/93). Its footer says it is prescribed by GSA/ICMR and cites FRMR (41 CFR) 201.5-202.1, and GSA hosts a copy in its forms library.
The form has three parts. Receipt of Body at Morgue records the name, the time and date the body was received, and a signature. Certificate of Removal records the name, the undertaker's name and address, the time and date of removal, and the signatures of the person releasing the body and the undertaker's representative. A physician's statement on the condition of the remains as released, which the form says shall be completed only when specifically ordered, includes a yes or no question on a medical implant that may include a battery or power cell.
A patient identification block with register and ward numbers ends the page. You can fill it out free in your browser on this page and download the result without an account.
