AO 425
Pre-Employment Information
AO 425, Pre-Employment Information, is a form from the U.S. Courts listed under Human Resources Forms. The current version is dated Rev. 11/92, and the U.S. Courts page lists it as effective on November 1, 1992.
The 2-page form is a personnel record, not a court filing. Section A identifies the person and names a requesting agency. Sections B to J then ask for federal employment status, retirement, grade and pay, appointment, unfavorable information, health and life insurance, service obligations, payroll and Thrift Savings Plan data, and the losing agency's release date. The form prints no instructions of its own.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.
