Form 433-A (OIC) (sp): Collection Information Statement, Spanish Version

Form 433-A (OIC) (sp) is the Spanish version of Form 433-A (OIC), Collection Information Statement for Wage Earners and Self-Employed Individuals. The IRS prints it in Spanish, under the title "Información de Cobro para los Asalariados y Trabajadores por Cuenta Propia", for taxpayers who read Spanish. The form says it should only be used with Form 656, Offer in Compromise, which it names as "Formulario 656, Ofrecimiento de Transacción".

It collects personal and household information, employment, personal assets in the U.S. and abroad, business assets, income and expenses for the self-employed, and monthly household income and expenses. It then walks through a calculation of the minimum offer amount. The sections, questions and notes are in Spanish, and the catalog number and footer "Form 433-A (OIC) (sp) (Rev. 4-2026)" are in English. Where the form sends a business owner to the business statement, it names Form 433-B (sp).

This is the April 2026 revision (Rev. 4-2026), dated "abril de 2026" in its header. The English edition is Form 433-A (OIC). The PDF can be filled out free in the browser on this page and downloaded without an account.

How to fill out Form 433-A (OIC) (sp)

  1. 1

    Sections 1 and 2

Enter personal and household information. Section 2 is for wage earners who receive a Form W-2. If you or your spouse own part of the employer's business, the form says to also complete and submit Form 433-B (sp).

  2. 2

    Value assets in Section 3

Many lines multiply the current market value by 0.8 and subtract the loan balance. Round to the nearest whole dollar and enter "0" instead of a negative number.

  3. 3

    Apply the form's deductions

The form subtracts $1,000 from bank account totals on line 1, $3,450 from the first vehicle on line 6b and, on a joint offer, from the second on line 6d, and an IRS deduction of $11,980 on line 7.

  4. 4

    Self-employed: Sections 4 to 6

The form says to finish these before Sections 7 and 8. You may average 6 to 12 months of income and expenses, or enter totals from a current profit and loss statement on lines 17 and 29.

  5. 5

    Section 7 expenses

For lines 39 and 45, the form says to list the full allowable standard even if you pay less, and current expenses on the other lines. It points to the IRS collection financial standards page, noting it is in English.

  6. 6

    Figure the offer and sign

Multiply Box F by 12 if you will pay in five or fewer payments within five months or less, or by 24 if you will pay in 6 to 24 months. Add Box A and Box B (if applicable), then sign and date Section 10.
