Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Russian Version)
Form 15080 (ru) is the Russian version of the IRS consent form offered to taxpayers at VITA/TCE tax preparation sites. The headings, the consent terms and the signature lines are all printed in Russian, which lets a taxpayer who reads Russian go through the consent in that language. The Russian title also spells out VITA and TCE as a program of free income tax help and tax consultations for older people. The English edition is Form 15080.
The form asks for consent to "глобальный перенос данных" and gives the English name, Global Carry Forward, in parentheses. The form says this allows TaxSlayer LLC, the provider of the VITA/TCE tax preparation software, to make the taxpayer's return information available to any volunteer organization in the IRS VITA/TCE program that the taxpayer chooses for next season's return. This edition is dated October 2026 and says the consent is valid until October 30, 2028.
The form says the taxpayer is not obliged to fill it out to use the site's return preparation services. You can complete the Russian PDF free in your browser on this page and download it, with no account.

The form says federal law requires that this consent form be provided to the taxpayer, and that signing it is not a condition of the site's preparation services.
The form has a name, signature and date line for the main taxpayer (основной налогоплательщик) and another for the secondary taxpayer (вторичный налогоплательщик).
The consent lets the return preparer enter a personal identification number (PIN) in the tax software on the taxpayer's behalf to confirm agreement with the terms.
Named in the form as the software supplier that makes the return information available to the volunteer organization the taxpayer chooses next year.
The form says consent is not needed for the VITA/TCE partner preparing the return this year. It helps only when the taxpayer visits another VITA or TCE partner using TaxSlayer next year.
A VITA or TCE site provides the form. The taxpayer, and a second taxpayer if there is one, choose whether to sign.
