Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Khmer Version)
Form 15080 (km) is the Khmer version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS publishes it for taxpayers who read Khmer and have their return prepared at a VITA or TCE volunteer site. The title, section headings, consent text and signature labels on the PDF are in Khmer. The header gives the date in Khmer as October 2025, and the footer prints Form 15080 (km) (Rev. 10-2025).
The form asks whether the taxpayer agrees to let TaxSlayer LLC, the provider of the VITA/TCE tax software, make the return information available to any volunteer site in the IRS VITA/TCE program that the taxpayer chooses to prepare next year's return. That way the next return starts with this year's data, wherever this year's return was filed. This edition says the consent is valid until 30 November 2027.
The English edition, Form 15080, has its own page. You can fill out this Khmer PDF free in your browser on this page and download the result without an account.

The form says a taxpayer who wants an earlier end date or a narrower scope will refuse consent instead of signing.
To consent, the first taxpayer enters a name, signs and dates the first line.
A second taxpayer enters a name, signs and dates the second line.
The Khmer text says the taxpayer has the right to receive a signed copy of the form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form is signed at the site that prepares the return. It prints no mailing address.
The form says it is not needed to use the site's services, and a signature obtained by making those services depend on consent makes the consent invalid.
The terms say the consent is valid until 30 November 2027. The federal disclosure adds that a consent with no period stated is valid for one year from the date of signature.
Consenting allows the preparer to enter a PIN in the tax software on the taxpayer's behalf to confirm agreement with the terms.
The form says the taxpayer can call TIGTA at 1-800-366-4484 if return information was disclosed or used improperly or without permission.

The Khmer text says no consent is needed for the VITA/TCE partner preparing this year's return. It helps only when the taxpayer goes to another VITA or TCE partner using TaxSlayer next year.
A VITA or TCE site gives the form to the taxpayer, and the taxpayer decides whether to sign it. The Khmer text says federal law requires that this consent form be provided to the taxpayer.
