Supporting Documents to Prove the Child Tax Credit (CTC) and Credit for Other Dependents (ODC) for 2021
Form 14815-A is a two-page IRS list of the documents to send to prove the Child Tax Credit (CTC) and the Credit for Other Dependents (ODC) for 2021. It refers throughout to "the year we're examining" and asks for copies, not originals, of the listed items for each dependent.
The top of the form has spaces for the name of the taxpayer and the taxpayer identification number. The rest is a numbered list of eight items: taxpayer identification number, U.S. citizen, national or resident status, relationship, residency, support, disabled dependent, student, and a child who is the qualifying child of more than one taxpayer. Several items say when not to send them, for example when the dependent was born in the United States.
The form says a document not in English needs a true and accurate English-language translation with the translator's name, address and telephone number. You can fill in the name and number free in your browser on this page and download the form without an account.

The dependent's Social Security card if an SSN was issued. The form notes the CTC needs an SSN valid for employment before the return's due date, including extensions, while the ODC may still be allowed.
Proof the dependent was present in the U.S. during the year, such as lease, school, child care or medical records, and birth, marriage, adoption or foster placement records showing the relationship.
Documents showing where the taxpayer and each dependent lived for the entire year, such as a lease, mortgage or real estate tax records, or school, benefits or medical records.
Documents and estimates of amounts paid for the dependent's support, how much came from each source, and declarations from others under a multiple support agreement.
Proof of permanent and total disability, full-time student records for any part of any five months, and residence records when the dependent is the qualifying child of more than one taxpayer.
Enter the name of the taxpayer and the taxpayer identification number at the top of page 1.
Each item says when not to send it, for example items 1 and 2 when the dependent was born in the U.S., and item 3 when the dependent isn't related to the taxpayer.
Collect copies of the items that apply for each dependent. The form says not to send original documents.
Add an English translation with the translator's name, address and telephone number for any document not in English. The form says otherwise the IRS may disallow the item.
Items 6 and 7 apply to a dependent over age 18 at the end of the year who was disabled or a full-time student. Item 8 applies when the dependent is the qualifying child of other taxpayers.
The form points to Publication 972, Publication 519 and Publication 501 for terms such as the Substantial Presence Test, student, and permanently and totally disabled.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
