Virtual VITA/TCE Taxpayer Consent (Haitian Creole Version)
Form 14446 (ht) is the Haitian Creole version of Form 14446, Virtual VITA/TCE Taxpayer Consent. The PDF carries the Creole title Konsantman Kontribyab pou VITA/TCE Vityèl, and the IRS made it for taxpayers who read Haitian Creole and get help at Volunteer Income Tax Assistance (VITA) or Tax Counseling for the Elderly (TCE) sites. The English edition is Form 14446.
The Creole opening says the form is required when part of the return preparation process happens without in-person interaction between the taxpayer and the VITA/TCE volunteer. It adds that the site explains its process to the taxpayer and, when needed, tells the taxpayer about the procedures and the risk of moving personal information from one site to another. The site fills in Pati I and Pati II, and the taxpayer answers Pati III and signs.
You can fill it out free in your browser on this page and download it without an account.

The form says IRS employees pick some free tax preparation sites at random to evaluate, the site corrects any errors found, and the IRS keeps no personal information from the reviewed return.

The site explains each of the 11 processes. Item 2 asks how the 14446 is signed, received and stored, and item 11 asks whether the return is e-filed right away or at the end of the day.
Check Wi or Non on letting an IRS employee review the return for accuracy if the site that prepares it is selected.
Check Wi or Non on using the site's virtual process, fill in name, date of birth, date, phone and email, then sign electronically or type or write the signature.
On a joint return, the spouse fills in the second column, which is labeled for a married person filing jointly, and signs.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form says page three (Paj twa) stays at the site with all other required documents. There is no IRS mailing address on it.
Consent is valid for the length of time the taxpayer states. If no length is given, the form says it is valid for one year from the date of signing.
The Creole text says the site may not be able to prepare the return through this process.
The form names the Treasury Inspector General for Tax Administration (TIGTA), by email at [email protected]. This edition prints the phone number as 1-800-366-44-84.
