Form 13614-C (ht): Intake/Interview and Quality Review Sheet, Haitian Creole Version

Form 13614-C (ht) is the Haitian Creole version of the Intake/Interview and Quality Review Sheet used in the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program. The PDF is printed in Haitian Creole, including the volunteer columns and the privacy notice. Its heading reads "Feyè enfòmasyon preliminè/entèvyou ak kontwòl travay ki fèt la". It is meant for taxpayers who read Haitian Creole and get help from an IRS-certified volunteer preparer.

This edition is Rev. 10-2026, and its questions refer to 2026, such as marital status as of December 31, 2026. The first page says to complete pages 1 to 4 and to bring tax documents such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN letters for everyone on the return, and photo ID for you and your spouse.
