Form 8975: Country-by-Country Report

Form 8975 is the IRS country-by-country report. The instructions say Form 8975 and its Schedules A (Form 8975) are used to report certain information about a U.S. multinational enterprise (U.S. MNE) group on a country-by-country basis each year.

The IRS says annual country-by-country reporting is required by certain U.S. persons that are the ultimate parent entity of a U.S. MNE group with annual revenue for the preceding reporting period of $850,000,000 or more. Form 8975 itself identifies the filer and the group. The constituent entities and the financial and employee information for each tax jurisdiction go on the attached Schedules A (Form 8975).

The instructions say the form is filed with the IRS together with the ultimate parent entity's income tax return, not separately. You can fill out the form free in your browser on this page and download the result without an account.
