Form 433-B (SP): Collection Information Statement for Businesses, Spanish Version

Form 433-B (SP) is the Spanish version of Form 433-B, Collection Information Statement for Businesses. The IRS prints it in Spanish, under the title "Información de Cobro Para los Negocios", for business owners who read Spanish. A business reports its details, the people who own and run it, other financial information, assets and liabilities in the U.S. and abroad, and a monthly income and expense statement.

Every page carries a notice that Executive Order 14224, Designating English as the Official Language of the United States, makes English the official language, so the English version of any document is the official version of all information the federal government publishes. The English edition is Form 433-B. Apart from that, the form is in Spanish, with a few English labels such as the "Section 4" heading, the Treasury header and the catalog number.

The form says to enter the most current data available or "N/A" in each space, and that leaving spaces blank could cause the request to be rejected or the account resolution to be significantly delayed. This is the June 2026 revision (Rev. 6-2026), dated "junio de 2026" in its header. The PDF can be filled out free in the browser on this page and downloaded without an account.
