Form 13615 (sp): Volunteer Standards of Conduct Agreement, Spanish Version

Form 13615 (sp) is the Spanish version of Form 13615, the Volunteer Standards of Conduct Agreement for the VITA/TCE Programs. The PDF is printed in Spanish under the title Acuerdo sobre las normas de conducta para los voluntarios, Programas de VITA/TCE, for volunteers who read Spanish. It is signed by volunteers and approved by a site official. Taxpayers who get help at a site do not fill it out. The English edition is Form 13615.

The form says the mission of the VITA/TCE programs is to help eligible taxpayers meet their tax responsibilities by providing free tax return preparation. It says all VITA/TCE volunteers pass the Volunteer Standards of Conduct certification and sign and date Form 13615 (sp) before working at a site, and that the form is not valid until the coordinator, sponsoring partner, instructor or IRS contact confirms the volunteer's identity, name and address with a government-issued photo ID and signs.

This edition is dated October 2025 (octubre de 2025). Page 1 lists six standards of conduct and the possible consequences, and page 2 holds the volunteer's details, certification levels, signatures and a continuing education section. You can fill it out free in your browser on this page and download it without an account.

How to fill out Form 13615 (sp)

  1. 1

    Complete the certifications

The form says volunteers pass the Volunteer Standards of Conduct certification, and roles that answer tax law questions also certify in Intake/Interview and Quality Review and tax law, before signing.

  2. 2

    Volunteer details

Print your full name and volunteer positions, check IRS Employee if it applies, and enter home address, email, daytime phone, sponsoring partner or site name and years as a volunteer, including this year.

  3. 3

    Certification levels

The form says to add the letter P for every exam passed. A volunteer with a professional designation (attorney, CPA, CTEC or enrolled agent) adds the licensing state and number.

  4. 4

    Sign and date

By signing, the volunteer declares completing the certification and agreeing to follow the standards, and certifies being a U.S. citizen, a legal resident, or otherwise residing legally in the United States.

  5. 5

    Approval

The approving official verifies the certification levels and a government-issued photo ID, then signs and dates. A parent or guardian signs when the volunteer is their child.

  6. 6

    CE credits, if requested

The coordinator or partner completes this section. The form says CE credits are not issued to enrolled agents, non-credentialed preparers or CTEC registered preparers who do not have a PTIN.

Before working at a site

The form is signed and dated before the volunteer works at a VITA/TCE site, and it is not valid until the approving official confirms the volunteer's ID and signs.

Sending CE requests

For CE credits, the coordinator, sponsoring partner or instructor signs and dates the form and sends it to the SPEC territory office or relationship manager for processing.

CE credit amounts

The form says a minimum of 10 volunteer hours is required to issue CE credits. Advanced qualifies for 14 CE credits, and Advanced and one or more specialty courses for 18.

Disclosure penalty

The form says a violation of IRC section 7216(a) is a misdemeanor, with a maximum of up to one year in prison or a fine of not more than $1,000, or both, plus the cost of prosecution.

Background checks

The privacy notice says sponsoring organizations may run background checks on their volunteers.

More information

The form points to Publication 4961 (sp) for the standards of conduct, and to Publications 5362 and 5683 for CE credit rules, noting that both are in English.

Return preparers, quality reviewers, coordinators, client facilitators and tax law instructors also certify in Intake/Interview and Quality Review and in tax law. So do greeters and screeners who answer tax law questions.

A coordinator, sponsoring partner, instructor or IRS contact checks the certification levels and a government-issued photo ID before the volunteer works at the site, then signs and dates.

  • Parents or guardians

A parent or guardian signs to give permission for their child to volunteer in the VITA/TCE programs.

  • Volunteers asking for CE credits

When an unpaid certified volunteer asks for continuing education credits, the coordinator or partner completes the CE section on page 2.
