Intake/Interview and Quality Review Sheet (Spanish Version)
Form 13614-C (sp) is the Spanish version of Form 13614-C, the Intake/Interview and Quality Review Sheet used in the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program. The whole PDF is in Spanish, under the title "Hoja de Admisión/Entrevista y Verificación de Calidad", so a Spanish-speaking taxpayer can answer the intake questions before an IRS-certified volunteer prepares the return.
This edition is dated October 2026 and its questions refer to 2026. It says to complete pages 1 to 4 and to bring tax information such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN letters for everyone on the return, and photo ID. The columns marked as completed by the certified volunteer are for the volunteer. Page 1 also asks whether you would like written IRS communications in a language other than English.
The PDF ends with Form 15080 (sp), the consent for Global Carry Forward. The English edition is Form 13614-C. You can fill out this PDF free in your browser on this page and download it without an account.
