Form SS-4 (sp): Spanish Application for Employer Identification Number (EIN)

Form SS-4 (sp) is the Spanish version of Form SS-4, Application for Employer Identification Number (EIN). The PDF is titled "Solicitud de Número de Identificación del Empleador (EIN)" and is dated "Rev. diciembre de 2025" (Rev. 12-2025). Every line label, the signature area and the "Do I need an EIN?" chart on page 2 are in Spanish. The English instructions for Form SS-4 point to this edition as the Spanish-language version.

The header says the form is for employers, corporations, partnerships, trusts, estates, churches, government agencies, U.S. Indian tribal entities, certain individuals and others. The IRS About page describes an EIN as a 9-digit number, for example 12-3456789, used for tax filing and reporting. The form says to see the separate line-by-line instructions at www.irs.gov/FormSS4SP and to keep a copy for your records.

The English edition, Form SS-4, has its own page. You can fill out this Spanish PDF free in your browser on this page and download it without an account.
