Application to Participate in the Income Verifcation Express Service (IVES) Program
Form 13803, Application to Participate in the Income Verification Express Service (IVES) Program, is a two-page application a business files with the IRS. The same form handles a new application, a renewal, an amendment, a new location, a cancellation or an address change.
It asks for the business's legal name, EIN or SSN, addresses, the reasons for using IVES (mortgage services, banking service or other), and details for the IVES account principal, a business point of contact and a responsible official. Line 14 is an agreement to review Publication 4557, Safeguarding Taxpayer Data, and to use taxpayer information received through a Form 4506-C request only for the purposes the taxpayer or requester intended.
The form says to fax the application to the IVES application line and that the IRS runs a suitability check before it notifies the applicant of acceptance. You can fill out Form 13803 free in your browser on this page and download it without an account.

Legal name of the business, EIN or SSN, DBA name, business location address with phone, fax and email, and a billing address.
The IVES account principal's name, title, phone, date of birth, SSN, email and home address, then the business point of contact.
Yes or no questions on felony convictions in the last 10 years and on whether everyone listed is current with tax filing and payments.
Estimated annual volume of IVES product requests, and the responsible official's details on page 2.
The Publication 4557 agreement box, the penalties of perjury declaration, and the name, title, signature and date of the IVES account principal.
Check the type of application, enter an existing IVES participant ID if one applies, check the organization status and select every reason for using IVES.
Enter the legal name and EIN or SSN, which the form marks required. Add a DBA name only if the business operates under a different name, and a billing address only if it differs from line 7.
Complete the IVES account principal's details, then a business point of contact if that person is different from the principal.
Answer both questions. The form asks for an attached explanation for a Yes on line 11a or a No on line 11b, and an EIN if a different one meets business filing requirements.
Enter the estimated annual volume of IVES product requests. If assigning a responsible official, the form says all line 13 fields are required.
Check the line 14 box, then have the IVES account principal type or print their name and title, sign and date. The form says electronic signatures are not permitted.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.

The form says to notify the IVES program at [email protected] within 30 days of any changes to the designated officials and the information on lines 4-13.
The form says acceptance is not transferable. If the business is sold or its organizational structure changes, a new application is filed.
In the declaration, the signer agrees to abide by the IVES rules and procedures and to pay resulting fees timely. The form does not print the fee amounts.
Partnerships, sole proprietorships, corporations, LLCs and other organizations, using IVES for mortgage services, banking service or another reason they specify.

Line 13 names an individual with responsibility for the operation and IVES users at the business location. A principal may also be the responsible official.
