Request for Appeal of Offer in Compromise
Form 13711 is the IRS form titled "Request for Appeal of Offer in Compromise". It works from the rejection letter: the form asks for the specific items the taxpayer disagrees with on the Income and Expense Table and the Assets and Equity table received with that letter, with a brief statement of why.
The form also covers disagreement with a reason for the rejection that is not discussed on the table, and with the Service's analysis of economic hardship or Effective Tax Administration. Supporting documents are attached for each disagreed item, and the form asks for a copy of the rejection letter if possible.
There is room for two taxpayers and an authorized representative, who attaches a completed Form 2848. You can fill it out free in your browser on this page and download the result without an account.
