Application for Employment Authorization for Abused Nonimmigrant Spouse
Form I-765V, Application for Employment Authorization for Abused Nonimmigrant Spouse, lets certain abused nonimmigrant spouses request an Employment Authorization Document (EAD). The USCIS page says it is for a person who is, or was, the abused spouse of a nonimmigrant admitted in A, E-3, G or H status, and who accompanied or later joined that spouse in the United States.
The instructions say the form is filed under INA section 106, added by the Violence Against Women and Department of Justice Reauthorization Act of 2005, when the applicant or the applicant's child has been battered or subjected to extreme cruelty.
USCIS says employment authorization is granted for a limited time, does not establish eligibility for or extend lawful status, and has no effect on immigration status. The form covers initial requests, replacement cards and renewals.

The instructions say to type unknown when an answer is not known, and to give some identifying details so USCIS can try to verify the spouse's status.
A stamped or typewritten name is not accepted, and the instructions say an unsigned application is rejected. This page lets you fill the form online free, then print it for signature.

The instructions describe battery or extreme cruelty by the nonimmigrant spouse, against the applicant or the applicant's child, during the marriage and after admission in qualifying status.
The instructions list current residence in the United States, and the form says Form I-765V is filed while in the United States. The instructions also say USCIS denies the application if the applicant remarries before a decision.
The form also covers renewing an EAD and replacing a lost, stolen or damaged card, or one with an error not caused by USCIS. The instructions say a card with a USCIS error is corrected by letter instead.
