Alternative Media Preference
Form 9000, Alternative Media Preference, is a one-page election for receiving IRS written communications in an accessible format. The IRS About page says to use Form 9000 to elect to receive written communications from the IRS in an accessible format.
The instructions say taxpayers with print disabilities can use it, and that the election applies to written communications that would otherwise be sent in standard print. Those include formal letters and notices sent to individual taxpayers about their income tax account, and other notices and correspondence. The IRS sends a standard print copy in the same envelope as the alternative media copy.
The form can go with an individual income tax return or be mailed by itself. This page hosts the November 2021 edition, with the instructions printed on pages 3 and 4. You can fill out Form 9000 free in your browser on this page and download the result without an account.
