Form 5434, Joint Board for the Enrollment of Actuaries Application for Enrollment, is the application an individual files to become an enrolled actuary. The declaration on the form reads "I hereby apply to be an enrolled actuary." The Privacy Act notice says the Joint Board for the Enrollment of Actuaries collects the information to enforce and administer its regulations governing the practice of an actuary under the Employee Retirement Income Security Act of 1974 (ERISA).

The applicant gives personal details, enrollment status, months of responsible actuarial and pension actuarial experience, how the basic and pension actuarial knowledge requirements were met (EA-1 and EA-2 exams or a waiver), and answers questions about tax compliance and conduct. Schedule A records each employer or period of actuarial employment within the last 10 years.

The form says the application fee is $680. Instructions are printed on pages 3 and 4. You can fill out the PDF free in your browser on this page and download it without an account.
