Form 3911 (sp): Taxpayer Statement Regarding Refund, Spanish Version

Form 3911 (sp) is the Spanish version of Form 3911, Taxpayer Statement Regarding Refund. The IRS prints it in Spanish under the title "Declaración del Contribuyente sobre el Reembolso" for taxpayers who read Spanish. The About page says Form 3911 is completed by the taxpayer to give the IRS the information needed to trace the nonreceipt or loss of an already issued refund check.

The form says to complete all of it if you did not receive your refund or if the refund check you received was lost, stolen or destroyed, and that parts that do not apply may be marked N/A. Its three sections, notes and certification are in Spanish. The Treasury header line, the catalog number and the footer "Form 3911 (sp) (Rev. 10-2022)" are in English. For how to complete it or where to send it, the form points to the Form 3911 About page and notes that the page is in English.

This is the October 2022 revision (Rev. 10-2022), dated "octubre de 2022" in its header. The English edition is Form 3911. The PDF can be filled out free in the browser on this page and downloaded without an account.
