Form 15086, Offer in Compromise Public Inspection File Request, is a one-page IRS form used to request a facsimile of Form 7249, Offer Acceptance Report. The form says the IRS will not respond to incomplete requests, and every contact field is required. You can fill out Form 15086 free in your browser on this page and download it without an account.
