Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Polish Version)
Form 15080 (pl) is the Polish version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS publishes it for taxpayers who read Polish and have their return prepared at a VITA or TCE volunteer site. The consent text on the PDF is in Polish, under the title "Zezwolenie na ujawnienie danych dotyczących rozliczenia podatkowego ośrodkom korzystającym z oprogramowania VITA/TCE". The header is dated Październik 2025 and the footer prints Form 15080 (pl) (Rev. 10-2025).
The form asks whether the taxpayer agrees to Globalne przenoszenie danych (Global Carry Forward). The Polish text says this lets TaxSlayer LLC, the provider of the VITA/TCE tax software, make the return data available to any site in the VITA/TCE program that the taxpayer chooses for next year's return. This edition says the consent is valid through listopada 30, 2027 r. (November 30, 2027).
The English edition, Form 15080, has its own page. You can fill out this Polish PDF free in your browser on this page and download the result without an account.
