Virtual VITA/TCE Taxpayer Consent (Persian/Farsi Version)
Form 14446 (fa) is the Persian (Farsi) version of Form 14446, Virtual VITA/TCE Taxpayer Consent. The PDF is printed in Persian, right to left, under the title رضایتنامه مؤدیان مالیاتی برای VITA/TCE مجازی, which reads as Taxpayer Consent for Virtual VITA/TCE. The IRS lists it as the Persian/Farsi Version, for taxpayers at VITA/TCE sites who read Persian. The English edition is Form 14446.
The Persian text says the form is needed when intake/interview and/or quality review are not arranged in person between the taxpayer and the VITA/TCE volunteer. The site fills in Part I and Part II. The taxpayer answers the review question in Part III, says whether they agree to the site's virtual process, and signs.
You can fill it out free in your browser on this page and download the result without an account.

Drop off (delivery) site, receiving site, a site for return preparation and/or quality review only, combination site, and the 100% virtual VITA/TCE process.
Eleven numbered items, each with guiding questions in parentheses: appointments, the taxpayer's consent, intake, identity checks with photo ID and Social Security cards/ITIN letters, interview, preparation, communication, quality review, sharing, signing and e-filing.
The form says IRS staff randomly review some free tax preparation sites, the site makes needed corrections, and declining does not change the services the site provides. The boxes are بله (Yes) and خیر (No).
Says that if the taxpayer consents to non-IRS virtual systems and they are hacked or breached, federal law cannot protect the return information from use or distribution. The sites are managed by IRS supported partners.
A Yes or No on using the site's virtual VITA/TCE process, then legible name in capital letters, date of birth, date, phone, email, and an electronic or typed/capital letter signature, for the taxpayer and the spouse.
The site enters its name, address, SIDN, coordinator, contact name and phone, then marks each virtual method, A to E, that it uses.
The site explains how it handles each of the 11 processes. Item 2 asks how the 14446 is signed, received and stored, and item 11 asks whether the return is e-filed immediately or at the end of the day.
Check بله or خیر on letting IRS staff review the accuracy of the return if the site that prepared it is selected.
Check بله or خیر on using the site's virtual process, enter name, date of birth, date, phone and email, then sign electronically or by typing or in capital letters.
On a joint return, the spouse fills in the second column, labeled for the spouse when a joint return is filed, and signs.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The Persian text says page three of the form is kept at the site together with all other required documents. No IRS mailing address appears on the form.
The disclosure says consent is valid for the period the taxpayer sets, or for one year from the date this form is signed when no period is set.
The Persian text says the site cannot prepare the return based on this process.

The consent disclosure says that if the return is prepared jointly for a couple, both people sign and date the document.
The note under method A says a taxpayer who leaves tax documents at the site and then leaves the site's property for any reason completes Form 14446. Waiting in another room or a vehicle is not a drop off.
