Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Persian/Farsi Version)
Form 15080 (fa) is the Persian (Farsi) version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS PDF is written in Persian for taxpayers who read Persian and are offered the consent at a VITA or TCE site. The English edition is Form 15080. This page describes the Persian edition in English.
The form asks the taxpayer to agree to Global Carry Forward. The Persian text says this lets TaxSlayer LLC, the provider of the VITA/TCE tax software, make the taxpayer's return information available to any volunteer site in the IRS VITA/TCE program that the taxpayer chooses for next filing season's return. This October 2025 edition says the consent is valid until November 30, 2027.
The form says completing it is not required to use the site's tax return preparation services. The primary taxpayer and any secondary taxpayer print their names, sign and date. You can fill it out free in your browser on this page and download the result without an account.

Explains that, unless the law allows it, return information is not shared with third parties for purposes other than preparing and filing the return without consent.
Describes how TaxSlayer LLC makes the return information available to a participating volunteer site the taxpayer chooses next year, and gives the November 30, 2027 end date.
Lists items such as name, address, date of birth, phone number, SSN, filing status, occupation, employer's name and address, income amounts and sources, deductions and credits, and the name, SSN, date of birth and relationship of all dependents listed on the return.
Two paragraphs on limiting the consent's duration and the scope of the disclosure. Each ends by saying the taxpayer will refuse consent in that case.
The consent statement, then lines for the primary and secondary taxpayer's printed name and signature, each with a date.
A note that the Treasury Inspector General for Tax Administration (TIGTA) can be contacted about improper disclosure or use at 1-800-366-4484 or through the TIGTA web address the form prints.
