Form 13989: IRS Tax Forum Case Resolution Data Sheet

Form 13989, IRS Tax Forum Case Resolution Data Sheet, is the sheet a tax practitioner completes to get an appointment with the Practitioner Case Resolution Program at an IRS Tax Forum. The instructions on page 2 say IRS representatives with specialized experience are available by appointment only to meet one-on-one with tax professionals to discuss a client's tax case, and that a case not resolved on site is assigned to an appropriate IRS professional for follow-up.

The sheet has three sections: the client taxpayer or business, the authorized representative, and a description of the problem. A box at the top is for IRS use only. The form ties every appointment to a Form 2848, Power of Attorney and Declaration of Representative, that lists the practitioner.

The instructions say the program cannot help with a practitioner's personal tax issue. You can fill out the PDF free in your browser on this page and download it without an account.

Getting an appointment and what to bring

Monday appointments

The instructions say to visit the Practitioner Case Resolution table in the general Forum Registration area from 2:00 p.m. to 7:00 p.m.

Tuesday to Thursday

The instructions say to go directly to the Practitioner Case Resolution Program. The Forum Information Desk can direct practitioners to the room.

Form 2848

The instructions require a physical, signed copy of Form 2848 even if one is already on file with the IRS, and say an appointment cannot be scheduled without it. For a joint return year, each spouse files a separate power of attorney.

One case per practitioner

The instructions say only 1 appointment for 1 case will be allowed per practitioner.

Cases not resolved on site

The instructions say a case that cannot be resolved at the Forum is assigned to an appropriate IRS professional for follow-up.

Privacy

The Privacy Act statement says supplying the information is voluntary, but leaving out information may limit the expediency, accuracy or completion of the assistance.

Who uses Form 13989?

  • Tax professionals at the Forum

The instructions invite practitioners to bring their toughest IRS case, one case per practitioner, to the Practitioner Case Resolution Program.

  • The representative on Form 2848

The form says only the practitioner listed will be allowed entry, and the instructions say only the individuals listed on the POA may enter the program.

  • The client taxpayer or business

Section 1 describes the client: the taxpayer or business name, address, and the tax periods and tax forms the case is about.

  • IRS case resolution staff

IRS staff fill in the For IRS Use Only box with the appointment number, day, time, table number and contact.

  • Not for personal tax issues

The instructions say the program cannot help with a personal tax issue and point to the IRS or the Taxpayer Advocate Service instead.

A tax professional attending an IRS Tax Forum completes the sheet for one client case and gives it to the Practitioner Case Resolution Program. It is not a form a taxpayer files for their own account.
