Form I-905: Application for Authorization to Issue Certification for Health Care Workers

Form I-905, Application for Authorization to Issue Certification for Health Care Workers, is filed by an organization, not by an individual worker. The instructions say its purpose is for an organization to apply for authorization to issue certificates to health care workers under section 212(a)(5)(C) or section 212(r) of the Immigration and Nationality Act.

The instructions describe the review: USCIS checks the application for completeness, then forwards a copy to the Secretary of Health and Human Services (HHS) for an opinion. After HHS gives its opinion, USCIS decides and sends the decision in writing. A denial comes with information on how to appeal.

An approval lasts 5 years. The instructions say the organization applies for reauthorization before that period ends, and USCIS reviews continued compliance with the standards along with that request.

How to fill out Form I-905

  1. 1

    Use the current edition

The USCIS page lists edition 04/01/24. On a printed copy, it asks that the edition date and page numbers stay visible on every page and that all pages come from the same edition, or the form may be rejected.

  2. 2

Answer every item in black ink

Type or print legibly in black ink. The instructions say to write N/A where a question does not apply and None where a numeric answer is zero.

  3. 3

    Address the standards

The instructions ask for a statement on how the organization meets the standards on structure, resources, candidate evaluation, duties to applicants, current information, fair examinations, awarding certificates and keeping accreditation.

  4. 4

Add extra sheets the right way

For more space, the instructions say to attach a sheet with the organization name at the top, the page, part and item number, and a signature and date on each sheet.

Where to file, copies and fees

Filing by mail

The USCIS page lists one address: Nebraska Service Center, P.O. Box 82521, Lincoln, NE 68501-2521. The instructions point to uscis.gov/I-905 for the most current filing information.

Two complete copies

The special instructions on the USCIS page say to submit two complete copies of this application.

Fee

The fee is on the USCIS Fee Schedule page (Form G-1055), which can change. Check it on the day you file.

Evidence and translations

Send legible photocopies unless an original is requested. A foreign-language document needs a full English translation with the translator's certification of completeness and competence.

Attorney or representative

An attorney or accredited representative who prepares the application files Form G-28 with it, and the box at the top of page 1 shows that Form G-28 is attached.

After approval

The instructions say an organization that revokes a certificate notifies USCIS through the Nebraska Service Center and the state regulatory authority, and reports pass rates of certificate holders to HHS each year.

Who files Form I-905?

  • Credentialing organizations

The instructions say an organization wishing to obtain authorization to issue certificates to health workers under section 212(a)(5)(C) or 212(r) of the INA files this application.

  • Occupations covered

The instructions list licensed practical, licensed vocational and registered nurses, occupational therapists, physical therapists, speech language pathologists and audiologists, medical technologists, physician assistants and medical technicians.

  • Independent organizations

Under the standards, the organization is incorporated as a legal entity and independent of any group that represents the occupation or is related to a recruitment or placement organization. One whose fees depend on whether an applicant gets a visa may not be approved.

  • Governing body make-up

The standards call for members from the same health care discipline who are eligible to practice in the United States, plus at least one voting public member who is not part of the discipline, with an equal say in credentialing matters.

  • Who signs

The applicant organization signs Part 2. The instructions say every application must contain the original signature of the applicant, and USCIS does not accept a stamped or typewritten name in place of a signature.

Questions about Form I-905

Can a nurse or therapist file Form I-905?

No. The instructions describe Form I-905 as an application by an organization for authorization to issue certificates to health care workers. It is not a worker's own application.

Which occupations does Form I-905 cover?

The instructions list nurses (licensed practical, licensed vocational and registered), occupational therapists, physical therapists, speech language pathologists and audiologists, medical technologists, physician assistants and medical technicians.

How long does an approval last?

The instructions say an approved application is valid for 5 years. The organization applies for reauthorization before it expires, and USCIS reviews compliance at that time.

The USCIS page lists a mailing address at the Nebraska Service Center and does not list online filing. It also says to send two complete copies.

What role does HHS play?

The instructions say USCIS forwards a complete application to the Secretary of Health and Human Services for an opinion, and makes its decision after HHS provides that opinion.
