Information Return for Federal Contracts
Form 8596 is an information return that certain federal executive agencies use to report information about persons with whom they have entered into contracts. The IRS About page says agencies use it when they are not filing electronically and have not made the Federal Procurement Data Center (FPDC) election. The form says it is submitted with Form 8596-A, Quarterly Transmittal of Information Returns for Federal Contracts.
The instructions say the head of every federal executive agency or their delegate files Forms 8596 and 8596-A to report federal contracts, under section 6050M and Regulations section 1.6050M-1. The contractor is the person being reported, not the filer: its name, address and taxpayer identification number go in boxes 1 and 2.
You can fill out Form 8596 free in your browser on this page and download the result without an account.

Name and employer identification number of the federal executive agency that entered into the contract.

Contract number, agency code, contract office number and contract modification number.
General instructions on page 1 cover purpose, how, when and where to file and who files. Page 2 defines a contract, lists 15 exceptions and gives box-by-box instructions.
Enter the full name and address of the contractor. For a sole proprietor, the instructions say to enter the sole proprietor's name first, then the business name (if any).
Enter the contractor's TIN. The instructions say that for an individual, including a sole proprietor, it is the social security number, and for all others it is the employer identification number.
If the contractor is in an affiliated group filing a consolidated return, enter the common parent's name and EIN. Then enter the agency's name and EIN. One central office filing for the entire agency uses only one name and EIN.
Enter the date of the contract action, or the date of the increase when reporting an increase. Enter the expected completion date, using any reasonable method such as the expected delivery date under the contract schedule.
Enter the total amount obligated to the contractor under the terms of the contract. When reporting an increase, enter only the amount of the increase.
Enter the contract number, agency code, contract office number and contract modification number, if available. The instructions say these help identify the contract if the IRS requests a levy.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
