Questionnaire—Filing Status, Exemptions, and Standard Deduction
Form 5129 is an IRS questionnaire about the filing status, exemptions for dependents and standard deduction on an income tax return. Section I asks for the name(s) and address exactly as shown on the income tax return, the tax form number and the tax year ending. Section II asks the taxpayer to check a box "to show how you intended to file your tax return."
The questions are about a return for a tax year the taxpayer names on the form. The IRS posts the form as a PDF only, with no About page and no separate instructions, and the form prints no mailing address, fax number or due date. This is the December 2007 revision, and it uses the terms printed on it, such as exemptions and qualifying widow(er).
The taxpayer signs under penalties of perjury, and the form says both spouses sign if filing jointly. You can fill out Form 5129 free in your browser on this page and download the result without an account.
