Form 15504: Supporting Documents for Clean Vehicle Credits

Form 15504 is an IRS request for information about the clean vehicle credit. The form says, "We need to verify the information reported on Form 8936, Clean Vehicle Credit, attached to your Form 1040, U.S. Individual Income Tax Return." It adds that IRS records indicate one or more vehicle identification numbers (VINs) reported on Schedule A (Form 8936) may be invalid or ineligible for the credit.

For each vehicle, the form asks for the VIN, copies of the seller report and the purchase agreement, and Yes or No answers to nine questions about the vehicle. It also prints a note that the Clean Vehicle Credit for vehicles acquired after September 30, 2025, was terminated under new legislation, and that vehicles acquired before September 30, 2025, but placed in service after September 30, 2025, may still be eligible for the credit.

The form comes with a letter that explains how to respond. You can fill out Form 15504 free in your browser on this page and download the result without an account.
