Where's My Refund
Form 13691 (EN-SP) is a short IRS sheet titled "Where's My Refund", printed in English on one half and in Spanish ("¿Dónde está mi reembolso?") on the other. It says: "You filed your tax return and are expecting a refund. Access the IRS Web site to find out if the IRS received your return and whether your refund was processed and sent to you."
The sheet has spaces to note the date the return was e-filed or sent by mail, and lists what is needed for refund status: the primary Social Security Number, the filing status on the return and the exact refund amount. It also prints a phone number, 1-800-829-1954.
The form prints no mailing address and no signature line. You can fill it out free in your browser on this page and download the result without an account.
